The Adult Transition Unit is designed to assist individuals with the following:

  • Auditory or visual hallucinations
  • Delusional or grandiose thinking
  • Hyperactivity
  • Hypervigilance
  • Poor impulse control
  • Paranoia
  • Inability to communicate
  • Self-harm or thoughts of self-harm
  • Intrusive thoughts
  • Substance Use

The program is a distinct treatment program designed for patients who are in an acutely disturbed phase of a serious mental illness. Care and treatment offered is recovery-oriented, trauma-informed, patient-centered, multidisciplinary, intensive, comprehensive, and collaborative and has an immediacy of response to critical situations.

  • Medication management to reduce reoccurrence

    • Substance use education
  • Collaboration with outpatient providers
  • Collaboration with outpatient providers
  • Aftercare planning for continued recovery

Patients and families or referring professionals can call SUN 24/7 to schedule a no cost assessment. Adults can be assessed and admitted directly to SUN. A SUN Intake Specialist will be waiting to help. Professional referrals are not necessary. Inpatient and outpatient services provided. Programs available for all ages.
